The Heart Failure Reaccreditation followed a thorough onsite review of the staff’s ability to evaluate, diagnose, and manage heart failure patients, covering areas such as pre-hospital care, early stabilization, acute care, transitional care, and clinical quality measures.
To earn Heart Failure Accreditation, hospitals complete a detailed clinical process that includes a gap analysis, care variance review, action plan development, onsite evaluation, and ongoing monitoring. Accredited facilities implement streamlined processes, guidelines, and best practices supported by a dedicated team of doctors, nurses, clinicians, and staff. These efforts lead to better patient education, improved outcomes, and more effective disease management.
