The Community Health Needs Survey for Clay County closes this Sunday, October 6th, and more public input is needed by the community. As part of the Affordable Care Act, the IRS requires hospitals to conduct a Community Health Needs Assessment every three years and implement strategies to meet the needs identified through the assessment.

This assessment is vital to help meet the needs of citizens in the area. This assessment evaluates preventative and treatment resources in the County, access to mental health resources, and more.
The goal of this survey is to help CCMC, the Health Department, and other entities identify what health needs should be prioritized in the community. Spending a few minutes to fill out the survey will help the County target programs and services based on the community’s input. The survey can be found at the top of CCMC’s Facebook page.
