FREE School and Sports Physicals

Family Physicians will be offering free school entry and sports physicals again for the upcoming school year.  Starting May 1 thru September 30, 2024 at all locations.

Call to make appointment at any of the locations:  Clay Center, Clyde, Linn, Riley and Glasco.

Prior to appointment, visit online at:  www.ccmcks.org/familyphyicianspatientforms.php and complete the Sports Physical Form (PDF).
