The Family Physicians Clinics of Clay County Medical Center (CCMC) will be providing free School Entry Physicals and Sports Physicals this summer from May 1st to September 30th at all locations.
Bryce Dolan, CCMC’s Marketing, Communications, and Foundation Director said School Entry Physicals can be scheduled at any time.
Sports Physicals cannot be scheduled prior to May 1st, per KSHSAA rules said Dolan.
To get School Entry Physical forms contact your student’s school. Sports Physical forms can be found on CCMC’s website under the Clinics Patient Forms tab and are fillable on the website, or you can pick up a form at any Clinic or your student’s school. Be sure to bring all completed forms and insurance cards to the appointment.
